The United States Dept. of Health and Human Services has a unit called the Substance Abuse and Mental Health Services Administration (SAMHSA). They have put together a facility locator for substance abuse treatment programs and providers. It is web-based and can do searches based on a radius centered on your ZIP code or by other criteria. The information is presented on a map along with a list of facilities. No identifying information is requested and they have a privacy pledge.
